Das Jahr 2007 war für die Mannheimer Universitäts-Augenklinik
der Ruprecht-Karls-Universität von einem Ausbau der
wissenschaftlichen Aktivitäten geprägt, die sich von der
Hornhaut im Vorderabschnitt des Auges bis zur Retina und
neuro-ophthalmologischen Fragestellungen im Augenhinterabschnitt
erstreckten.
Wie
auch in den vorangegangenen Jahren befand sich die Mannheimer
Universitäts-Augenklinik im Jahr 2007 unter den drei
wissenschaftlich führenden deutschen Universitäts-Augenkliniken,
gemessen an Publikationen und Zitationsrate.
Seit 2000 ist in Mannheim die Forschungsleistung auf mehr als das
25-fache angestiegen. Hierfür gebührt allen Mitgliedern der
Mannheimer Augenklinik für ihren Einsatz und Engagement und
Herrn Geschäftsführer Dänzer für ein
reibungsloses Miteinander Dank. Von Mannheim aus erfolgten mehr
internationale ophthalmologische Publikationen als von mehr als einem
Viertel der deutschen Universitäts-Augenkliniken zusammen. Führt
man einen internationalen Vergleich durch, nimmt die hiesige Klinik
in Europa nach dem weltweiten Spitzenreiter Moorfields Eye Hospital
in London einen der ersten 20 Plätze ein, obwohl die Mannheimer
Augenklinik von der Anzahl der ärztlichen Mitarbeiter die
kleinste bundesdeutsche Universitäts-Augenklinik mit gesamt zwei
ärztlichen wissenschaftlichen Mitarbeitern ist, die auch für
die Studentenausbildung zuständig sind.
Im
Jahr 2007 nahm die wissenschaftliche Zusammenarbeit mit dem Beijing
Institute of Ophthalmology, dem Suraj Eye Institute in Nagpur /
Indien, dem L.V. Prasad Eye Hospital in Hyderabad / Indien, und dem
Singapore National Eye Centre weiter zu. Das Beijing Institute of
Ophthalmology ist Forschungszentrum des Tongren Eye Hospital, eines
der drei größten und bekanntesten Augenkliniken in China.
Die Ergebnisse der zusammen mit dem Beijing Institute of
Ophthalmology durchgeführten epidemiologischen Beijing Eye Study
bezogen sich auf den gesamten Bereich der klinischen Ophthalmologie
und auf Fragestellungen über den Bereich der Augenheilkunde
hinaus, wie zum Beispiel der Häufigkeit und den Risikofaktoren
der arteriellen Hypertonie und des Diabetes mellitus, und dem
Zusammenhang zwischen Körpergröße, Bildung, Einkommen
und Vorkommen von okulären und systemischen Erkrankungen.
Zusätzlich wurden grundlagenwissenschaftlichen Studien zur
intraokulären Anwendung von Stammzellen und tierexperimentelle
Studien zur klinischen rekombinanten intraokulären
Zytokinsynthese und zur Neuroprotektion (Ginkgo biloba) durchgeführt.
Seit der Zusammenarbeit mit der Mannheimer Augenklinik hat sich
innerhalb von 4 Jahren die Anzahl der internationalen Publikationen
des Beijing Institute of Ophthalmology mehr als verzehnfacht, so dass
der Mannheimer Klinikdirektor zum Honorary Director des Beijing
Institute of Ophthalmology ernannt wurde. Basierend auf den Erfolgen
der Beijing Eye Study wurde gleichzeitig der Präsident des
Beijing Institute of Ophthalmology, Herr Prof. Liang Xu, als ein
Vertreter für Public Health in Chinas „National
Negotiating Committee“, einer Struktur parallel zum
Zentralkomitee der Kommunistischen Partei gewählt.
In
Kooperation mit dem Suraj Eye Institute in Nagpur / Indien wurden in
der Central India Eye and Medical Study inzwischen mehr als 3000
Studienteilnehmer bezüglich ophthalmologischer, internistischer,
psychiatrischer und allgemeiner sozio-ökonomischer
Fragestellungen untersucht. In Zusammenarbeit mit dem L.V. Prasad Eye
Hospital in Hyderabad / Indien, einer der drei klinisch und
wissenschaftlich führenden Augenkliniken Indiens, wurden
histomorphometrische Untersuchungen durchgeführt als auch die
Whole-Genome-Analysis der Proben der Central India and Medical Study
begonnen. In Kollaboration mit dem Singapore National Eye Centre
erfolgten Untersuchungen zur klinischen Glaukomdiagnostik. Neben
diesen wissensschaffenden Aktivitäten führte Frau Prof.
Kreissig weltweit Teaching Kurse für Netzhautchirurgie mit bis
zu 700 Teilnehmern pro Tagung durch, wofür sie die Goldmedaille
der Algerian Ophthalmological Society erhielt.
Die
wissenschaftlichen Aktivitäten der Mannheimer
Universitäts-Augenklinik beinhalteten die Weiterentwicklung der
intraokulären medikamentösen Therapie von
Makulaerkrankungen, wesentlich der alterassoziierten
Makuladegeneration, des diabetischen Makulaödems, der
proliferativen diabetischen Retinopathie und retinaler
Venenverschlüsse. Die weltweit erstmals berichtete intravitreale
Knochenmarkstransplantation mit dem Ziel der Regeneration der
Netzhautgefäße und der Makula wurde ebenso wie
tierexperimentelle Untersuchungen zur intraokulären
rekombinanten DNA-gestützten Medikamentensynthese durchgeführt.
Das letztgenannte Verfahren wird möglicherweise im Jahr 2008
klinisch zur Behandlung von Netzhauterkrankungen eingesetzt werden.
Basierend auf experimentellen Voruntersuchungen wurde in der
Hornhautchirurgie der Femtosekundenlaser zur gestuften Schnittführung
eingesetzt, wodurch der postoperative Astigmatismus im Transplantat
deutlich gesenkt und dadurch der Visus verbessert werden konnte. Im
Bereich der Kataraktchirurgie wurde die Möglichkeit der Hypnose
als Alternative zur topischen Tropfanästhesie untersucht. In der
Glaukomchirurgie wurde Bevacizumab als Anti-Angiogenetikum zur
postoperativen Hemmung der Filterkissenvernarbung eingesetzt und die
Rolle des Hirndruckes als eigentlichem Gegendruck zum Augeninnendruck
werter untersucht. Neuro-ophthalmologisch zeigte sich die
Ophthalmodynamometrie als geeignet, in bestimmten Situation den
Hirndruck nicht-invasiv zu erfassen.
Die
Ziele für das Jahr 2008 beinhalten neben der Fortsetzung der
unten skizzierten wissenschaftlichen Projekte den Aufbau eines
Verbundnetzes von „Nanotechnology in Medicine (Ophthalmology)“.
Für
den klinischen Alltag und für die Ergebnisse der
wissenschaftlichen Studien ist es mir eine Freude und Ehre, allen
Klinikmitarbeitern in Mannheim und den Mitgliedern aller
kooperierenden Kliniken und Institute herzlich zu danken.
